COLLECTIVE MEMBERSHIP APPLICATION AND AGREEMENT

COLLECTIVE MEMBER TERMS & CONDITIONS

This Collective Member Terms & Conditions Agreement (“Agreement”) outlines the complete terms between you (“Member”) and MY GREEN LIBRARY (the “Collective”). PLEASE READ THIS AGREEMENT CAREFULLY BEFORE JOINING AS A MEMBER. You are STRONGLY ADVISED to consult the legal status of medical cannabis in your jurisdiction and obtain professional legal advice before signing this Agreement. Some California law enforcement or courts may not recognize this Agreement as valid. YOU ARE RESPONSIBLE FOR RESEARCHING AND UNDERSTANDING YOUR LOCAL LAWS AND THE LEGALITY OF THIS AGREEMENT BEFORE SIGNING. YOU ACKNOWLEDGE AND ASSUME ALL RISKS ASSOCIATED WITH MEDICAL CANNABIS USE.

You agree to indemnify and hold MY GREEN LIBRARY, its affiliates, officers, agents, and employees harmless from any loss, liability, or claim, including attorney’s fees, arising from your activities as a Collective member. AS A CONDITION OF MEMBERSHIP, YOU, YOUR HEIRS, AND THOSE WITH YOU EXPRESSLY DISCLAIM ALL WARRANTIES, INCLUDING THE WARRANTY OF MERCHANTABILITY AND THE WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE. KEEP ALL CANNABIS PRODUCTS SECURELY STORED AND AWAY FROM CHILDREN OR ANYONE ELSE. FAILURE TO DO SO IS AT YOUR SOLE RISK AND RESPONSIBILITY.

DECLARATION UNDER PENALTY OF PERJURY:

  1. I have been diagnosed with a serious illness for which cannabis provides relief, and I have received a recommendation or approval from a licensed California physician.
  2. I understand that my contributions for cannabis are used to sustain the Collective’s operations and do not constitute commercial promotion.
  3. Payments made are solely to help the Collective continue its operations, maintain staff, cover expenses, and provide members with medical cannabis.
  4. I designate the Collective and its representatives as my agents to assist in obtaining legally prescribed medical marijuana. I authorize them to possess, purchase, cultivate, transport, and distribute medical cannabis exclusively for members.
  5. I authorize the Collective to contact my physician to verify my cannabis recommendation.
  6. I agree to rely on the Collective as my exclusive source of medical cannabis.
  7. This designation remains effective for 12 months, until the expiration of my physician’s recommendation, or until revoked in writing.
  8. I authorize the Collective to create or assign agency rights in its own name to grow cannabis for my personal medical use and for other members.
  9. I acknowledge that all cannabis possessed by the Collective is collectively owned by its members, subject to Collective rules and guidelines.
  10. I verify that I am a California resident and that my medical cannabis will not leave the state or be shared, sold, or exchanged with others.
  11. I understand that compliance with the Collective’s rules is the responsibility of all members, including myself. Violations may result in immediate termination of membership.
  12. Prohibited Activities: Engaging in any prohibited activity will result in immediate termination of this Agreement and your membership without notice. Prohibited activities include, but are not limited to:
    • Possessing or transporting more cannabis plants than legally allowed.
    • Transporting cannabis outside of California.
    • Misrepresenting yourself as an employee or representative of the Collective.
    • Possessing illegal narcotics or paraphernalia.
    • Violating any section of the California Penal Code or local ordinances.
  13. I understand and assume all risks associated with cultivating medical cannabis. I acknowledge that this Agreement and my status as a grower may not be legally recognized in all jurisdictions.
